The G7 is made up of the world's seven largest influential societies and advanced economies - the UK, USA, Canada, Japan, Germany, France and Italy.
Footage shared online shows leaders posing for a photograph on the lawn in France, before Trump tries to walk off in the wrong direction.
He is then told "this way" and looks around before being guided in the correct direction. As he turns to head the right way, he gives a thumbs up to the camera man.
Throughout this reaction, the president has a bewildered look on his face.
One social media user wrote: "A confused Trump wanders off in the wrong direction and has to be guided back in front of G7 world leaders."
Another added: "Trump starts wandering off in the wrong direction after a G7 photo and world leaders have to step in and redirect him"
A third commented: "This is the US President at an international meeting, babbling, looking lost, and with balance and mobility issues. He’s unfit."
Speculation surrounding the president's health have escalated recently despite Trump claiming that his recent medical exam went well.
His well-being is often scrutinized with regular clips and images of the 80-year-old going viral online.
Trump took to his social media platform Truth Social to announce he'd finished a physical exam, concluding that "everything checked out perfectly".

In recent weeks, as well as being spotted snoozing during meetings, the US leader has posted duplicate posts on his social media and confused nations with one another, igniting fears about his cognitive health.
Trump has already caused controversy at the summit, appearing to blank prime minister Sir Keir Starmer as he greeted other world leaders at the G7 summit.
Footage aired on GB News showed Starmer standing awkwardly beside Trump, who is enthusiastically shaking hands with others without acknowledging him.
The presenter claimed that the prime minister was "brutally blanked" by Trump as they stood in close proximity.
Further footage from the summit has also seen Trump criticised.
He was caught having an awkward exchange with Brigitte Macron in a video that emerged showing him engaging in a "weird handshake tug of war" with her.
